Description
The County's Emergency Medical Services Operational Program (EMSOP) is comprised of the St. Mary’s County Department of Emergency Services – Emergency Medical Services Division, the St. Mary’s County Rescue Squad Association, the St. Mary’s County Fireboard Association, and the Jurisdictional Medical Director (JMD). The JMD functions in accordance with all local, state and federal regulations and reports to the Director of Emergency Services. The JMD shall provide EMS Operational oversight in accordance with the provisions set forth in COMAR 30.03.03.03 and is responsible for overseeing the quality of patient care provided by the EMSOP. The JMD shall provide expert medical opinions on the development of EMS operational policies, making recommendations for clinical improvements related to standards of patient care. They shall provide medical direction and oversight of the department’s Emergency Medical Dispatch (EMD) program, suggesting operational improvements to the Director of Emergency Services. The JMD serves as a liaison with local, state and regional partners regarding Emergency Medical Services systems and is responsive to the needs of first responders and the community.
MAXIMUM 10 HOURS PER WEEK
May conduct or direct medical-related research necessary for making recommendations related to the development or approval of policies, procedures, medical devices, clinical skills, medications or emergency medical dispatching to improve pre-hospital patient care.
Responsible for the department’s Quality Assurance/Quality Improvement Program. They shall conduct audits, review trends and assist the EMS Chief or designee in root cause analysis for clinical process improvement and improved patient outcomes.
Develop, implement and monitor a program for reviewing complaints and protocol violations. Review any protocol variations, making recommendations for corrective action.
Provide oversight of EMS training programs. Review and approve curricula for remedial and continuing education programs and participate in delivery of clinical, didactic or remedial training of EMS clinicians.
Develops and maintains a process to credential and approve EMS providers, reviews and approves applications to MIEMSS for licensure, recertification and maintenance of affiliation.
Serve as a liaison between St. Mary’s County EMS and the local medical community as well as regional and State Medical Directors.
Serve as an advocate for first responders and community health.
Review the Maryland Medical Protocols for EMS Providers annually and provide a written summary report to the Director of Emergency Services after each annual review.
Routinely work with the EMSOP's leadership to interpret and implement best patient care practices, which may include:
Implement optional supplemental protocols;
Develop and implement pilot protocols;
Other programs as needed (local and statewide).
Assist with the development and implementation of system-wide performance measurements.
Conduct EMS patient care case reviews of significant incidents as a vital part of the Quality Assurance process.
May respond to emergency incidents to observe the care being provided and provide pre-hospital emergency care as appropriate.
May ride along with EMS units to observe and gain familiarity with operations and assigned personnel.
Provide medical oversight of controlled substance program including ordering, storage, distribution, use and audits in compliance with DEA Federal Regulations. Conduct an annual review of department policies on controlled substances, making recommendations to the Director of Emergency Services for process improvement.
Provide guidance on the department’s Infection Control Program. May provide consultation on vaccination and testing protocols.
Serves on the EMS Medical Review Committee.
As part of the QA/QI process, may suspend or limit a provider’s privileges, provided the State EMS Medical Director, MIEMSS Chief Compliance Officer and Director of Emergency Services are notified immediately. Filing appropriate documentation with MIEMSS Office of Integrity.
Consistent with COMAR30, approve the devices, equipment and supplies utilized by EMS providers.
As necessary, conduct audits of EMS field reports to determine compliance with protocols and accepted medical practice and standards of care. Monitor community outreach and CPR programs and make recommendations to the EMSOP with regards to performance and improvements as needed.
In addition to the standard field operations, the JMD may provide medical oversight for special operations programs that support Tactical EMS Teams in collaboration with the St. Mary’s County Sheriff’s Office.
May participate and represent the Director of Emergency Services, St. Mary’s County EMS or the EMSOP at interagency, MIEMSS, State of Maryland or other regional meetings.
As requested, may provide expert medical opinion in litigation issues that affect St. Mary’s County.
Performs other duties required for improvement and enhancement to public and responder safety.
Possess current knowledge of the Maryland EMS System.
Familiarity with all applicable local, state, and federal laws and regulations pertaining to pre-hospital emergency medicine.
Knowledge of COMAR Title 30.
Knowledge of Maryland Medical Protocols.
Ability to direct disaster and mass casualty plans and operations.
Knowledge of Medical quality assurance processes inclusive of both pre-hospital field operations and 911 communications center emergency medical call taking and dispatch.
Knowledge of Emergency medical dispatch protocols.
Specialized training in pre-hospital Emergency Medicine.
Ability to complete the MIEMSS Quality Assurance Officer Course.
Ability to complete the MIEMSS Base Station Provider Course.
Knowledge of Basic & Advanced Disaster Life Support.
Knowledge of Tactical Emergency Casualty Care (TECC) or Tactical Combat Casualty Care (TCCC) for pre-hospital providers.
Use of hospital services and medical systems, including St. Mary’s County and surrounding jurisdictions.
Quality Management for public safety systems.
Specialized skills, techniques, tools, and practices required to deliver emergency medical care during special operations (Tactical EMS, Advanced Hazmat Life Support, WMD/CBRNE incidents & Wilderness Medicine).
Importance and integration of a Critical Incident Stress Management (CISM) Program.
Ability To:
Develop and maintain a professional working relationship with volunteer and career leadership.
Develop a professional mentoring relationship with volunteer and career EMS providers in the EMSOP.
Develop a professional working relationship with MIEMSS, including the Region V Medical Director, the State Medical Director, and the Executive Director.
Establish a positive working relationship with local hospital administrators and staff that accept patients delivered by the EMSOP.
Assist the EMSOP in the research, development, and implementation of modern, forward- focused optional local protocols.
Chair and direct EMSOP Quality Assurance Meetings and Medical Review Board Meetings.
Conduct meetings and briefings for elected officials in an organized, clear, and concise manner.
Possess an active Maryland Controlled Dangerous Substance Registration Certificate and Federal Drug Enforcement Agency certificate.
Must possess an active, unrestricted license to practice medicine in the State of Maryland.
Must hold current Federal DEA and Maryland CDS licenses, or be able to obtain these within 3-6 months of hire.
Must possess a valid, unrestricted motor vehicle driver’s license.
Must meet the minimum criteria to be a Jurisdictional Medical Director as established by the Maryland Institute for Emergency Medical Services Systems (MIEMSS) to include:
Familiarity with the design and operation of EMS operational programs and systems, including emergency medical dispatch (EMD) and communications.
Experience in and knowledge of emergency care of patients who are acutely ill or injured.
Possess current knowledge of Maryland EMS system, including:
Applicable laws and regulations
The Maryland Medical Protocols for Emergency Services Providers
Applicable Emergency Medical Dispatch Protocols
Disaster and mass casualty plans
Organization and structure
Medical quality assurance process
Certification in EMS by the American Board of Emergency Medicine is preferred.
Seven years or more of related professional experience in emergency medicine, or additional education in a specialized area pertinent to out-of-hospital emergency medicine;
Experience as a jurisdictional medical director or assistant jurisdictional medical director is preferred.
Supervision:
Reports to the Director of Emergency Services.
Physical and Environmental Conditions:
Work requires light physical effort in the handling of light materials or boxes and tools or equipment in non-strenuous work positions up to 30 pounds and/or continual standing or walking of 60%/+ of the time.
Work environment involves everyday risks or discomforts that require normal safety precautions typical of such places as offices or meeting rooms, e.g., safe use office equipment, and/or avoidance of trips and falls.
Work may involve high risk in emergency operations, field clinical settings.
